Diagnosing Low Back Pain

The diagnosis of lumbar back pain is difficult and uncertain due to the various conditions which can present with this problem. Effective back pain management depends on identifying what kind of back pain problem is present, and many people have suggested that there are many back pain subtypes which need to be identified before treatment can be well targeted. The variations in diagnoses for low back pain and related symptoms include: postural pain; trigger point pains; nerve root compression; neuropathic pain; facet joint pain; disc related pain and lumbar stenosis.

The spinal facet joints, intervertebral discs, muscles and ligaments are all potential sources of mechanical back pain, a pain derived from the damaged or injured tissues and transmitted by the nervous system. When the nervous system is damaged or injured it can start generating pain itself, leading to the production of what is termed neuropathic pain. Typical diagnoses of this kind of pain are post-shingles pain, phantom pain, nerve root damage pain and diabetic neuropathy pain. Patients suffer badly with this kind of unpleasant pain and it is difficult to treat.

A recent study performed by researchers from Massachusetts General Hospital in Boston and Addenbrooke’s Hospital in Cambridge, UK, has investigated this difficulty. They recognised that the assessment by taking a score of pain intensity does not reflect the reality of the complex nature of pain processes by which pain is generated. They set out to design an assessment which would take these complexities into account, allowing the clearer identification of the diagnosis and thereby a potentially more accurate treatment. They developed a standardised tool to use in the assessment of chronic pain with the aim of delineating differing pain subtypes.

130 people with peripheral neuropathic pain and 57 people with mechanical low back pain were surveyed and given a standardised assessment. An interview with 16 questions was then applied followed by a specific series of twenty-three physical tests. A list of words applicable to pain descriptions was provided and patients were asked to indicate which ones most accurately described their pain. In chronic pain patients often have an alteration in the ability to feel touch, vibratory and pin ***** stimuli so the ability to discriminate these sensibilities is tested.

In neuropathic pain patients it was possible to identify six sub-groups and in non neuropathic patients two further subgroups were noted. Researchers were also able to distinguish the 6 questions and 10 physical tests which were best suited to making the most accurate discrimination between the pain subtypes. Testing this tool on one hundred and thirty seven further patients allowed the researchers to see it worked effectively and that patient acceptability was good. A particular group of neuropathic pain subtypes could be elucidated by a relatively low number of signs and symptoms which were not related to the presenting causative conditions.

The recording of the symptoms was less sensitive in distinguishing the neuropathic nature of the pains than the physical examination. The pain quality was less important than often noted and the pinprick testing more helpful. The researchers tried to link the pain subtypes with specific underlying biological mechanisms, with spontaneous pain of a burning nature linked to spontaneous discharges in heat sensitive pain nerves and pain from brushing related to increased sensitivity of cells in the dorsal horn of the spinal cord.

The physical examination was more sensitive in delineating neuropathic diagnoses of pains than the recording of the types and nature of symptoms.  The qualities of the pain were less helpful and the testing of pinprick more helpful. The researchers attempted to connect the underlying neural mechanisms with the pain subtypes. The heat sensitive pain nerves were linked to burning pains of a spontaneous type and heightened sensitivity of the spinal cord dorsal horn cells was linked to increased pain from brushing over the skin.

So what causes an asthma attack or even what is asthma? The airways between your mouth/nose and lungs are overly sensitive to one or more things when you have asthma. 1. Your airways have linings on the inside which become swollen and very irritable. This leaves less room for air to get back and forth to your lungs when you breathe. It also causes you to cough.

2. The muscles around the outsides of your airways start to get real tight. They try to squeeze your airways shut. This makes your airways smaller, which means that there is even less room for air to get through to your lungs.

3. The insides of your airways start to fill with what I call gunk or what others might call mucus or phlegm. This starts to plug up your already small airways which makes it even harder to breathe. Every time you cough, some of this gunk comes out.

Every asthma attack you have can be different. So sometimes those 3 bad things can be very mild and other times they can be very severe. They might also happen very slowly one time and very quickly another time. How long those things last can also be different every time. They might stop when you are no longer exposed to your triggers or they might last several days.

Those 3 things are all happening to your airways or breathing tubes. Once they start to get bad enough, then you will start to have what are called your typical asthma symptoms. These are the things that we associate with having an asthma attack and they are listed below:

1. Coughing – with your now irritated and sensitive airways, you will be coughing quite a bit. And there will be gunk coming up when you do.

2. Wheezing – this is sort of a whistling sound, usually when you breathe out. This is from your airways being much smaller than normal.

3. Tightness in the chest – this can sometimes be painful also. It is from the muscles around your airways getting so tight.

4. Shortness of breath – you just can’t take a normal breath while those 3 things are happening.

When those 3 things happening to your airways are bad, then these asthma symptoms will also get bad. It is not unusual for them to get so bad that you can’t breathe. The medications in your emergency asthma inhaler try to reduce your symptoms so that you can breathe normally again.

Asthma symptoms are pretty much the same for all asthma types except for cough variant asthma. With this type of asthma, your only symptom will be a very bad cough. And this cough will be a dry cough with none of the gunk being present. It can also be a very loud cough that is sometimes referred to as a barking cough. And the worst thing is that this cough can sometimes last for weeks. Because there are no other asthma symptoms present, this type of asthma often goes undiagnosed.

Knowing what is happening during an asthma attack should help you to keep from panicking. Getting stressed out can make your symptoms even worse. It is important to do what you can to help someone who is having an attack to try and stay as relaxed as possible.

If you notice that your asthma inhaler isn’t helping you breathe and your other symptoms aren’t getting any better either, then you might be having an acute asthma attack. If that happens, then it is very important that you seek immediate medical attention. Don’t wait and hope that it will get better. An acute asthma attack can be a life and death situation.

My Moo the Mighty Dog

I hung out with my old dog Mukunda, today, as I do every day.

I took him to my friend Ellen’s house and we exchanged the Mechano treatment, only after we tossed a few rounds of ball and marveled at how well Muki ignored Aslom, the cat.

We complimented him on his good manners. I took him for several leisurely walks around Ellen’s large city yard and we admired the spring crocus, and snow drops between dark green chive.

During our treatments, Mukunda laid dutifully on the floor, looking our way occasionally with a laughing pant.

He ate all of Aslom’s cat food. He had the run of that big old house and the yard, but he only wanted to be with us.

Later, I took him to the Vets. Muki has a tumor on the left side of his body which has doubled in size since December.

Frank, our Vet, had a look of concern on his face as he examined and palpated and probed. He thought that Muki looked awfully fat but in reality, had only gained 4 pounds since the last visit.

Was it all the cat food he ate earlier?

Was it all the late night winter snacking?

Frank wondered, Could it be ascites?

Ascites is an ominous sign. The mere mentioned raised my pulse rate.

Ascites is when the abdomen fills with fluid accompanied by a cancer that has metastized.

But Muki didn’t have a heart murmur, a good thing.

Before Frank would do the surgery, he did a chest xray and some blood work to determine the health–or illness–of my old dog.

Holding him down on the xray table was a challenge, because he kept nipping the technician. Drawing blood was no picnic either. He snarled a few times, but I whispered in his ear how much I love him.

Naturally he calmed down because above all else, Muke loves the word “love.”

Muke and I were ushered to the waiting room to wait for results when our Great White Purinese neighbor, Baxtor, came in for an appointment with his mistress to assess his allergies.

Muki and Baxtor immediately recognized each other and did a dance around the other.

Muki journeys to see Baxter every morning and evening as a sacred ritual to begin and end the day.

So as I waited for the test results, inwardly knowing I must face any bad news that Frank might have, but also knowing that Mukunda is alert, loves to eat, loves to sprint to the edge of the property several times a day, (while at the same time, bothered by the fast growing tumor), so I did not have an ominous feeling about what news Frank might have about the test results.

As much as I love my Mukunda, and as much as I don’t want him to die, I know the time will one day come when he will die.

And I will join him the moment I draw my last breath.

Frank came out and said, You have to see this xray!! It is gorgeous!!

And, indeed it was: clear lungs, strong heart, succinct borders around the liver and abdomen. Wow!! All those Mechano treatments I give him are paying off!! as well as meals made from scratch, sprinkled with brewers yeast and dried stinging nettle that grows prolifically along the mighty Conestoga River bank that spans the front of our property.

And Muke regularly meditates with the humans in the house.

He knows what “meditate” means.

Then later, the blood work result revealed no elevation of white cells, perfect liver enzymes, platelets excellent, Calcium within normal range, and an above average hemoglobin level.

Frank, I said, There’s one more very important consideration. Mukunda and I have never been apart. I take him on every vacation, I’ve never boarded him, and I work nights so he is always with another human being.

He needs to know that I will not desert him during his operation.

May I stay with him while he undergoes surgery?

Without hesitation, Frank said, Of course, Kate. He is your baby.

And I secretly wondered, How will I detach from my lovely beast?
